Use the following information to complete your application:

     We are the Leo Club of Sussex

     Include your email address with your home address

     Include your parent or guardian's email address with their address

     The Annual Dues are $10.00

     You will need to purchase either

                   a short-sleeve Leo t-shirt for $10.00, or

                   a long-sleeve Leo t-shirt for $15.00

     The parent or guardian signature should also include the parent or guardian email address.
